Maxillary premolars rarely presents with developmental defects and variations. These teeth pose a challenge for Endodontists for successful outcome. This can be achieved with the help of proper diagnosis and treatment planning followed by three most important steps in root canal treatment (access opening, cleaning and shaping followed by obturation). These types of teeth can be successfully treated with the use of recent advanced equipments such as Endomicroscope and Cone Beam Computed Tomography (CBCT).
